Resident Services Coordinator/Case Manager

We are seeking a Resident Services Coordinator/Case Manager to assist in the senior living buildings we have throughout our Chicago Housing Authority (CHA) portfolio.

Responsibilities:

  • Collaborates with the Resident Services team to plan, develop, coordinate, and implement activities, programs, and services to meet the educational, social, and community needs of residents.
  • Assesses the needs of each community and its Resident Association and develops a Community Workplan with the residents and an Individual Service Plan (ISP) with the resident and/or parent/guardian.
  • Makes appropriate referrals to agencies/service providers to meet the needs of community residents association officers.
  • Promotes and recruits resident participation in activities, in programs, and services.
  • Maintains appropriate files and records such as Program files and Service Provider files.
  • Conducts inspections with residents of housekeeping issues.
Requirements:
  • Ability to effectively relate and interact with people of diverse cultural, social and economic backgrounds.
  • Working knowledge of various resources and service providers.
  • Excellent verbal/written communication skills and leadership/ motivational skills.
  • Experience working with senior citizens and individuals with disabilities.
  • Experience in conflict resolution and crisis intervention.
  • Bachelor's degree in Social Work or related field required
  • Must be bilingual in Spanish

Ronald McDonald House Charities – Chicagoland & Northwest Indiana (RMHC-CNI) is an independent public charity governed by a local board of directors operating in the Chicago metropolitan area. The mission of RMHC-CNI is to care for families of children with complex medical needs by providing comfort, compassion and a sense of community. RMHC-CNI keeps families of hospitalized children together in a 'home away from home'.

With the current Chief Executive Officer retiring in 2015, RMHC-CNI is seeking a proven leader who can build its presence in a dynamic marketplace amid evolving healthcare regulations. The candidate should possess a proven track record of success, organizational growth and leading diverse teams. Key relationships will include hospital CEO’s, McDonald’s network and co-ops, donors, volunteers and of course the entire staff/volunteer team.

Substantial experience and ability to organize and lead a professional staff group is required. The application of an organization-wide, delegation-driven management approach is desirable. Staff mentoring, coaching and nurturing skills, particularly applied to early-career young professionals, are valuable traits to possess. Experience in building broad constituent networks that extend the brand and purpose of an organization is highly desirable. This would include evidence of innovation and collaboration with other institutions and enterprises in developing programs that increase the impact and reach of the organization. A substantial background working as, and with, volunteer leaders is essential.

The ideal candidate will show success in building the capacity of an organization by increasing public support from such sources as individual major gifts, endowed gifts, corporate partnerships/sponsorships and cause-related marketing.

Extensive knowledge and experience in the Metropolitan Chicago corporate and philanthropic community is highly desirable. Active membership in Chicago-area civic groups and professional associations would be a plus. Attainment of a bachelor’s degree is expected; an advanced degree is a plus.

Job Description
The Family Outreach Manager serves primarily to assist the efforts of UCSN to connect parents to the schools' work, students' academic achievement, and the network's self improvement and development programming for the adult community.

The Family Outreach Manager understands that they serve as a conduit from UCSN to the school community, including parents and other external partners.

Core Responsibilities Strategic Management
  • Carries out the mission of UCSN and its vision for parent engagement by designing and directing the work of network-wide programming that is focused on building parents' knowledge of their child's school and the school's work to promote parental development and understanding of the UCSN academic model.
  • Sets strategy to ensure that each school's enrollment targets are met by employing student recruitment efforts that focus on attracting a group of diverse learners from a variety of neighborhood communities with a wide range of academic abilities.
  • Builds and uses a performance management system with clearly defined metrics to evaluate progress of the work of the entire family outreach program. Through data analysis of these metrics, the Family Outreach Manager makes strategic decisions to improve student recruitment, parent engagement, and community outreach efforts
Recruitment
  • Creates a recruitment plan that outlines the work, roles, and responsibilities with a timeline of events and tracking system for recruiting new families to the schools and new community partnerships to the network.
  • Creates and directs strategy to recruit community partners and parent volunteers for assisting teacher and administrative needs at the schools and student recruitment from all Chicago communities including targeted strategy for recruiting diverse learners.
  • Develops and uses a performance management system to forecast enrollment and assess the effectiveness of student recruitment efforts.
Family Outreach Coordinators Development and Management
  • Manages a team of Family Outreach Coordinators.
  • Recruits, trains, develops, and coaches the Family Outreach Coordinators to offer a wide-range of skills and services.
  • Manages work allocation, professional development, problem resolution, and performance evaluation of an effective and dynamic team.
  • Builds capacity in Family Outreach Coordinators to provide information and other services to parents in an effort to access all of the school’s resources and information to better the student achievement, performance, and access to high school and college. For example: applications for services, translation services, etc.
  • Manages the Family Outreach Coordinators work to build site based programming, create school newsletters in partnership with school administration, provide translation services to schools, and create necessary communications to be sent to parents.
  • Develops protocol for Family Outreach Coordinators to assist with parent/teacher meetings and to connect with appropriate community agencies to provide services for students’ academic and social emotional needs.
Communications
  • Develops a system for communicating with UCSN executive leadership, UCSN central office team, UCSN schools, and UCSN families.
  • Develops a system by which Family Outreach Coordinators regularly communicate with school administration.
  • Sets expectations for Family Outreach Coordinators to communicate with parents on behalf of the school to build and facilitate the ongoing partnership between the home and the school.
  • Directs parent meetings on behalf of the school administration to gain information and stakeholder feedback and communicate key information to parents about the academic program and the network.
Parent Leadership Development and Engagement
  • Builds parent leadership program to develop leadership capacity and train parents to become key leaders in the school community.
  • Provides workshops and trainings to build the capacity of English language learners’ parents to support English language learner achievement.
  • Organizes various activities to foster parent and community engagement including fundraisers, festivals, volunteer days, student showcases, and book fairs.
  • Organizes workshops and activities to develop parents in areas of their self interest of self development like architectural landscaping, computer skills, culinary arts, small business ventures, etc.
Community Engagement
  • Develops relationships with community leaders and community organizations to provide resources and support to the schools.

Degrees and Professional Experience

  • Proven track record of creating and implementing innovative solutions to meet organizational needs
